Apply for Support

DEADLINE:
Inclusion Support Request Form and Participant Questionnaire must be completed and submitted a minimum of 10 business days (not including weekends and holidays) in advance of the start of the class or program in order to process your request. Evaluation of request for services will not begin until ALL documentation are submitted.
 
Step 1: Register for the Program of Your Choice

Before you request Inclusion Support, sign up for the program you are requesting support for.

Visit the registration page to sign up for a program of your choice.

   
IMPORTANT:
You must be registered for a program before proceeding to Step 2.
 
Step 2: Request Support

To request support, first fill out the Inclusion Request Form.

Please note:

The Inclusion Request Form must be filled out for each program/class/activity that the participant has signed up for.

Please note:
The Participant Questionnaire only needs to be filled out one time per year. If you have already filled it out this year, you can skip this step.

 
Step 3: What’s next?
Once you submit all the required paperwork, you will receive an email confirmation that your paperwork has been received. Staff will review your paperwork and contact you regarding your request.
